Photo & Media Release

PLEASE READ CAREFULLY — THIS IS A PHOTO & MEDIA RELEASE AGREEMENT

1) Parent/Guardian Certification

I certify that I am the parent or legal guardian of the child(ren) named in this registration and that I have the full legal authority to grant the permissions described in this Photo & Media Release.

2) Authorization for Photo and Video Use

By selecting “I Agree”, I authorize the Spruce Grove Barracudas Swim Club to capture, record, reproduce, and use photographs and/or video footage of my child(ren) during the 2026 swim season and any official club-sanctioned events or activities.

This authorization includes the right to use such images or recordings for marketing, promotional, educational, and public-relations purposes, including but not limited to:

  • The club’s official website
  • Social media accounts (e.g., Facebook, Instagram, YouTube, etc.)
  • Printed materials, digital brochures, posters, event programs, sponsor packages, and newsletters
  • Internal presentations, displays, and fundraising materials

3) Terms and Conditions of Use

I understand and agree that:

  • Participants will not be identified by name in any publicly shared photo or video.
  • Photos and videos may be edited, cropped, reformatted, or combined with other media as needed for club use.
  • All photographs and video recordings taken are the property of the Spruce Grove Barracudas Swim Club.
  • The club may use these images without financial compensation, royalties, or additional permissions.
  • The release applies to all media collected during the 2026 season unless revoked in writing before media use.

4) No Obligation to Use Images

I understand that the club is not obligated to use any image or recording of my child(ren), and that images may be removed or archived at the club’s discretion.

5) Withdrawal of Consent

I understand that I may withdraw consent at any time by submitting a written request to the club executive.
I acknowledge that withdrawal cannot apply retroactively to materials already created, published, or distributed.

6) Electronic Consent and Online Execution

By submitting this form online:

  • I agree that my electronic signature and selection of “I Agree” have the same legal effect as a handwritten signature.
  • I confirm that the release was presented clearly, that I had sufficient time to read it, and that I may contact the club with questions before agreeing.

7) Acknowledgment and Agreement

By submitting this form, I acknowledge that I have read and understood the terms of this Photo and Media Release and that I voluntarily consent on behalf of my child(ren).
